      Mitral annular calcification (MAC) is a degenerative process that occurs mostly in older patients, especially in women. This process is also more common in patients with end-stage renal failure. Mitral valve dysfunction and the other complications including atrial fibrillation, thromboembolic events and atrioventricular blocks increase in proportion to the intensity of calcification. We herein present a mitral annular calcification that twodimensional transthoracic echocardiography showed massive mitral annular calsification while three-dimensional transesophageal echocardiography didn't detect any severe mitral dysfunction.       Mitral anüler kalsifikasyon çogunlukla yaslılarda özellikle de kadınlarda görülen mitral kapagın dejeneratif degisikligidir. Son dönem böbrek hastalıgı olanlarda da sık gözlenir. Kapak disfonksiyonu ve atriyal fibrilasyon, tromboembolik olaylar ve atriyoventriküler blok gibi diger komplikasyonlar mitral anüler kalsifikasyonun yogunlugu ile orantılı olarak artmaktadır. Biz burada iki boyutlu ekokardiyografinin yogun mitral anüler kalsifikasyon görüntü vermesine ragmen üç boyutlu transözefajial ekokardiyografide kapakçıklarda ciddi anormallik yapmayan mitral anüler kalsifikasyon olgusunu tanımladık.
