Les conséquences de s'occuper du patient souffrant des troubles mentaux - définitions et instruments d'appréciation. (French)

Severe mental illnesses have far-reaching consequences for both patients and their relatives. This paper reviews literature on the measures of caregiving consequences. Authors provide a condensed knowledge and research results in the area of caregiving consequences, especially both subjective and objective caregivers' burden. The consequences of care apply to carers' social and leisure activities, financial status, health condition. The burden of care has three fundamental causes: the reorganisation of mental health services, a social isolation of patients and their families and the lack of systemic support for caregivers. The problem of caregiving consequences has been investigated in several studies. In order to identify factors, which have impact on caregiver distress, a variety instruments have been developed. This paper focuses on questionnaires useful for the systematic assessment of both objective and subjective burden: Involvement Evaluation Questionnaire (IEQ), Perceived Family Burden Scale (PFBS), Zarit Caregiver Burden Scale (ZCBS), Experience of Caregiving Inventory (ECI), Family Problems Questionnaire (FPQ). Les troubles mentaux sévères causent les conséquences importantes pour les patients ainsi que pour leurs familles. Ce travail donne la revue de littérature concernant surtout les instruments d'appréciation des conséquences de soigner ces patients. Les auteurs présentent l'état actuel des connaissances en question et les résultats des recherches concernant les conséquences de s'occuper des patients psychiatriques y compris les subjectifs et objectifs fardeaux de personnes soignant ces patients. Les conséquences touchent leur : fonctionnement social, loisirs, état financier et état de santé. Les fardeaux liés avec les soins des malades résultent de trois causes principales : réorganisation du système des services médicaux, isolation sociale des patients et de leurs familles, manques du système de soutient des familles. Le problème de conséquences a été examiné plusieurs fois, on a créé beaucoup d'instruments servant à identifier les facteurs influant sur le niveau du stress des personnes soignant les malades. Ce travail parle surtout des questionnaires mesurant les fardeaux subjectifs et objectifs tels que : Involvement Evaluation Questionnaire (IEQ), Perceived Family Burden Scale (PFBS), Zarit Caregiver Burden Scale (ZCBS), Experience of Caregiving Inventory (ECI), Family Problems Questionnaire (FPQ). Ernsthafte psychische Störungen implizieren weit gehende Folgen sowohl für die Patienten als auch für ihre Familien. Die vorliegende Studie stützt sich auf die Literaturübersicht zu den Werkzeugen, die zur Messung der Folgen der Betreuung dienen. Die Autoren beschreiben den aktuellen Stand und Ergebnisse der Studien an den Folgen der Betreuung der psychiatrisch kranken Patienten, darunter auch an den subjektiven und objektiven Belastung der Betreuer. Die Folgen der Betreuung betreffen die soziale Funktionsweise, Aktivitäten in der Freizeit, finanzielle Lage und Gesundheitszustand der Betreuer. Die berufliche Belastung, die mit der Betreuung verbunden ist, hat drei Ursachen - es ist die Reorganisation der psychiatrischen Gesundheitsbetreuung, soziale Isolierung der Patienten und ihrer Familien, auch die Mängel im System der Unterstützung der Patienten. Das Problem der Folge der Betreuung wurde schon mehrmals untersucht. Es wurde eine Reihe der Werkzeuge zur Messung gebildet, die zum Ziel haben, die Faktoren zu identifizieren, die einen Einfluss auf den Dysstressspiegel der Betreuer haben. Die vorliegende Studie fokkusiert auf die Fragebögen, die für systematische Beurteilung der Belastung brauchbar sind, sowohl der objektiven, als auch der subjektiven Belastung: Involvement Evaluation Questionnaire (IEQ), Perceived Family Burden Scale (PFBS), Zarit Caregiver Burden Scale (ZCBS), Experience of Caregiving Inventorz (ECI), Family Problems Questionnaire (FPQ).
