The article offers information on the papers submitted to the special series of the journal "Pediatrics" entitled "Promoting Lifelong Health for Adolescents and Young Adults With Special Health Care Needs." The first two paper focuses on the conditions at opposite ends of the transition spectrum where Simon and colleagues explain hydrocephalus as a prototypical condition in need of transition services. Furthermore, each paper offers a review of the transition from pediatric to adult-centered care for young adults and adolescents with a particular health condition which begins in childhood.
