      The author comments on a case report by M. K. Lee and colleagues about coronary artery anomaly (CAA) published within the issue. The medical case involved a 15-year-old boy with an anomalous left coronary artery who underwent eight episodes of syncope before being diagnosed with CAA. According to the author, the evidence presented in the report did not firmly support the claim that the distal left main coronary artery was narrowed beyond its intramural segment. Intravascular ultrasonography (IVUS) is cited as the best method to use in such stenosis.
