      The article discusses the clinical diagnosis of a young patient with a perianal rash that did not respond to the prescribed topical clotrimazole. It is confirmed that the child has perianal streptococcal dermatitis which presents as a bright red, moist, sharply demarcated perianal rash. The condition also includes itchiness, rectal pain and blood-streaked stools. The condition commonly found in children between six months to ten years of age. The culprit of the condition is the group A beta-hemolytic streptococcus which penetrates the anus through auto-inoculation. It is indicated that the intake of penicillin V or amoxicillin for ten days is effective as a first-line treatment for the perianal streptococcal dermatitis.
