Elementos para la formulación de estrategias: DE EXTENSIÓN DE LA PROTECCIÓN SOCIAL EN SALUD.

A pesar de las reformas emprendidas en los sistemas de salud durante los últimos veinte años, en la actualidad un porcentaje importante de los habitantes de los países de América Latina y el Caribe se encuentran excluidos de los mecanismos de protección social frente al riesgo de enfermar o las consecuencias de estar enfermos. A pesar de ello, la reducción de la exclusión en salud no aparece en forma explícita en las agendas políticas de salud de los países de la región. Este trabajo analiza las causas de la situación de exclusión en salud hoy en día en la región y propone un marco analítico para abordar la formulación de estrategias destinadas a extender la protección a partir de dichas causas, entendiendo la extensión de la protección social en salud como la garantía que la sociedad otorga, a través de los poderes públicos para que un individuo o grupo de individuos, pueda satisfacer sus necesidades de salud a través del acceso a bienes y servicios de manera adecuada, sin que la capacidad de pago sea un factor restrictivo. Se estima que la realización de este ejercicio permite tener mayor claridad respecto de la situación de exclusión en salud y de las intervenciones que han demostrado ser más eficientes para combatirla y visualizar mecanismos innovadores para extender la protección social junto a los esquemas ya existentes. Este trabajo pretende ser un aporte al conocimiento social que formula estrategias de protección, a través de la introducción de elementos analíticos que en la actualidad no son parte de la batería de instrumentos con que cuentan la mayoría de los responsables de la elaboración de políticas de salud de los países de la región. Despite of the reform processes undertaken in the past twenty years, a high proportion of Latin America and the Caribbean's inhabitants are nowadays excluded from the social mechanisms created to protect citizens from the risk and consequences of illness. Notwithstanding, the reduction of exclusion in health does not explicitly appear in the health sector political agenda in most countries of the Region. This work analyses the issues underlying the current situation of exclusion in health and introduces an analytical framework to approach the formulation of strategies aimed to extend social protection in health, taking exclusion in health as the starting point of the analysis and defining extension of social protection in health as the guarantee that society grants through public authorities to all individuals or social groups so that they can meet their health needs and satisfy their health demands by the means of adequate access to health care services, goods and opportunities regardless of their ability to pay. The authors hope this exercise will help to better understand the situation of exclusion in health and the interventions that have shown to be more effective to tackle it as well as to visualize innovative mechanisms to extend social protection in health along with the already existing schemes. This work intends to be a contribution to the social knowledge that will allow devising strategies of protection in health through the introduction of analytical tools that are not currently part of the toolkit available for the policy makers in the region.
