- Abstract:
With а high degree of stenosis or occlusion of the proximal (before the separation of vertebral artery) segment of subclavian artery, the blood supply to the ipsilateral upper extremity is from the vertebrobasilar system via pendular or retrograde blood flow in the ipsilateral vertebral artery thereby bypassing the stenotic area. This hemodynamic condition is known as subclavian steal syndrome (SSS). There are different variants of SSS, determining the clinical symptoms, which can vary from oligo- to pronounced symptoms due to manifestations with symptoms of vertebro-basilar insufficiency - recurrent vertigo, drop attacks, reduced hearing, tinnitus, positional nystagmus and headache, as well as symptoms of hypoperfusion of the corresponding upper extremity - numbness, trophic disorders, changes in arterial pressure and pulse rate. Colorcoded Duplex Ultrasound (CDUS) is a basic noninvasive screening method that could differentiate the SSS variant and provide comprehensive information on the hemodynamic characteristics of blood flow in the vertebral and carotid arteries. We present a systematic analysis on the topic and the clinicalpractical application of CDUS in a series of clinical cases from our practice. [ABSTRACT FROM AUTHOR]
