      A 74-year-old woman developed Pneumocystis jirovecii pneumonia (PJP) leading to hypercalcaemia while being treated with prednisone and methotrexate for large granular lymphocytic (LGL) leukaemia. She presented with confusion, weakness, and bilateral leg oedema, along with moderate hypercalcaemia and acute renal failure. Treatment with calcitonin, zoledronic acid, antibiotics, and fluids led to clinical improvement and normalization of calcium levels. The case highlights the importance of monitoring for potential complications when using prednisone and methotrexate in patients with LGL leukaemia. [Extracted from the article]
