- Abstract: Abstract: We report the case of a patient presenting with typhoid fever after returning from a stay in India. This infection was not cured with a course of ciprofloxacin, due to a reduced susceptibility of the bacteria to the drug. This decreased susceptibility to fluoroquinolones was not detected by the antibiogram, but the MIC for nalidixic acid was greater than 32 mg/l. This case suggests using a third generation cephalosporin instead of a quinolone, for people coming from a high-risk zone. It also suggests that the MIC for nalidixic acid and for norfloxacin can be used as the first clue for a reduced susceptibility to fluoroquinolones. [Copyright &y& Elsevier]
- Abstract: Résumé: Nous rapportons le cas d''''un patient ayant présenté une fièvre typhoïde au retour d''''un séjour prolongé en Inde. Le traitement de cette salmonellose par la ciprofloxacine a été un échec à cause d''''une diminution de sensibilité de la bactérie à cette molécule. Cette moindre sensibilité à la ciprofloxacine, n''''a pas été détectée sur l''''antibiogramme, mais il existait une CMI élevée pour l''''acide nalidixique. Ce cas suggère la nécessité d''''utiliser en première intention une céphalosporine de 3e génération injectable si le patient revient d''''une zone à risque d''''acquisition de salmonelles résistantes. Il suggère aussi la prise en compte de la résistance à l''''acide nalidixique ou mieux encore celle à la norfloxacine comme le premier indice d''''une éventuelle diminution de sensibilité aux fluoroquinolones.
