      A 70-year-old man with hepatocellular carcinoma metastatic secondary to hepatitis B virus infection experienced lack of efficacy during treatment with tenofovir-alafenamide and sorafenib. The patient presented with weight loss, abdominal pain, transaminitis, acute kidney injury, and HBV infection. Further investigation revealed advanced progression of the disease, classified as barcelona clinic liver cancer stage-C, with tumor thrombus metastases in multiple locations. While receiving treatment, the patient developed chest pain and dyspnea, potentially indicating pulmonary thromboembolism caused by tumor thrombi originating from the right atrium.
