      This study aimed to design a web-based medical record information system for a clinic in Bligo, Sidoarjo, to replace the manual processing of medical record data and transactions. The Waterfall method was used, starting with planning, designing, coding, and testing, while the system design used a structured programming approach. The resulting information system can handle all data related to medical record data, master and transaction data, and user access rights. The implication of this study is that the new system can improve the efficiency and accuracy of medical record data processing in the clinic, leading to better patient care and outcomes.
