      A federally qualified health center obtained accreditation for a diabetes self-management education and support (DSMES) program. A retrospective descriptive study was conducted to evaluate the outcomes of patients assigned to a registered nurse (RN)-led panel using DSMES standards and patients assigned to a primary care provider (PCP)-led panel. Patients assigned to the RN-led and PCP-led panels had significant improvements at 6 months in their HbA1c measures. The RN-led panel of patients had fewer emergency department visits. The revenue for nurse visits covered the RN's direct salary expense. [ABSTRACT FROM AUTHOR]
