      The article presents a case study of a 40-year-old woman who presented at the emergency department for a fever, rash, and arthralgia. The patient met the American Heart Association's revised Jones criteria for the diagnosis of rheumatic fever, but she did not respond to treatment. The second diagnosis of Sweet syndrome was made that responded positively to treatment. The patient was placed on penicillin V 250 mg twice daily, leading to improvement of papular rash.
