      Résumé: La majorité des cancers gastriques est issue de lésions pré-néoplasiques qui apparaissent plusieurs années auparavant. Ces lésions sont la dysplasie et, plus improprement pour les anatomo-pathologistes, l'atrophie et la métaplasie intestinale de la muqueuse gastrique induites le plus souvent par une infection ancienne à Helicobacter pylori. L'atrophie au niveau du corps gastrique associée à la métaplasie intestinale est un facteur de risque particulier de cancer. Il existe un effet bénéfique de l'éradication de H. pylori sur l'évolution des lésions pré-néoplasiques n'ayant pas atteint le stade de la métaplasie intestinale. Dans ce cas, une surveillance tous les trois ans est recommandée particulièrement chez les patients ayant des lésions étendues dans le corps gastrique et l'antre ou sévères quelle que soit la localisation. En cas de dysplasie de bas grade découverte sur des biopsies étagées, une surveillance annuelle est préconisée. The majority of gastric cancer is originated from pre-neoplastic lesions that appear in the previous years. These lesions are dysplasia and, more improperly for pathologists, the atrophy and intestinal metaplasia of the gastric mucosa most often induced by an old Helicobacter pylori infection. Atrophy of the gastric body associated with intestinal metaplasia is a particular risk factor for cancer. There is a beneficial effect of eradication of H. pylori on the development of preneoplastic lesions which have not reached the stage of intestinal metaplasia. In this case monitoring every 3 years is recommended particularly in patients with extensive lesions in the gastric body and antrum or severe lesions whatever the location. In the case of low-grade dysplasia discovered on biopsies, annual endoscopic monitoring is recommended. [ABSTRACT FROM AUTHOR]
