This case report describes clinical evolution of a 9-year-old patient diagnosed with scarlet fever associated with cholestatic hepatitis. The correlation is a situation of significant rarity, manifesting with favorable evolution after clinical treatment and benzathine penicillin use for streptococcus eradication. It is important to know scarlet fever atipic form manifestation and progress, with good prognosis in most cases, helps clinical decision by doctors, avoiding unnecessary and invasive interventions. O relato de caso descreve a evolução do quadro clínico de um paciente de 9 anos de idade com diagnóstico de escarlatina em associação à hepatite colestática. A correlação é uma situação de raridade significativa, se manifestando com evolução favorável após tratamento suportivo e uso de penicilina benzatina para erradicação de estreptococco. A importância do conhecimento dessa forma atípica de manifestação da escarlatina e da evolução com bom prognóstico na maioria dos casos, orienta melhor a condução clínica do quadro pelos médicos, muitas vezes evitando intervenções desnecessárias e invasivas.
