    • Abstract:
      The evolution of infections is influenced by the substrate on which they appear, the same etiologic agent being able to determine asymptomatic infections in some people, while in others it may produce severe manifestations and sometimes death. This variety of clinical manifestations is often induced by previous comorbidities of the patient or by his lifestyle (for example, people with low social status or the homeless ones). In addition to nutritional deficiencies and disorganized lifestyle, people with low socioeconomic status have preexisting chronic diseases such as diabetes, chronic obstructive pulmonary disease or infections. We present the case of a female patient, smoker, with low socioeconomic status, hospitalized for fever, chills, dyspnea and chest pain, diagnosed with pleural empyema and multiple lung abscesses, with favorable outcome under treatment.       Evoluția infecțiilor este influențată de substratul pe care se instalează, același agent etiologic fiind capabil să determine la unele persoane infecții asimptomatice, în timp ce la altele poate avea manifestări grave, uneori chiar spre un prognostic sever și deces. Această varietate de manifestări a tabloului clinic este indusă de multe ori de comorbiditățile anterioare ale pacientului sau de modul de viață al acestuia (spre exemplu, persoanele cu un status social nefavorabil sau cele fără adăpost). Acest grup are de multe ori, pe lângă lipsurile nutriționale și modul de viață dezorganizat, diverse patologii preexistente, boli cronice, precum diabet, bronhopneumopatie cronică obstructivă, sau boli infecțioase. Prezentăm cazul unei paciente fumătoare (caz social) internată pentru febră, frisoane, dispnee și junghi toracic, diagnosticată cu empiem și abcese pulmonare multiple, cu evoluție favorabilă sub tratament.
